Patient portal
Secure, HIPAA-compliant access to your chart.
Our practice utilizes a secure, HIPAA-compliant electronic health record system managed via our advanced HITRUST information technology infrastructure. Through the portal you can safely and securely:
- Review upcoming appointment details
- Send non-urgent medical inquiries directly to our clinic staff
- View published laboratory results, vital sign records, and clinical summaries
- Access billing statements and payment receipts
(Note: While appointments are currently scheduled by phone, online self-scheduling through the patient portal will be introduced in a future phase.)

Preparing for your visit
In-person clinic visits (Celina, TX)
Arrival Time
Please arrive 15 minutes before your scheduled appointment to finalize check-in and verify your information.
Identification & Payment
Bring a valid government-issued photo ID, your insurance card (if applicable), and an accepted payment method (credit card, debit card, HSA, or FSA card).
Medication Lists
Bring a complete list of all current prescription medications, over-the-counter drugs, vitamins, and supplements, including exact dosages.
Diagnostic Lab Prep
If your provider has ordered fasting blood work, do not consume food or beverages (other than water) for 8 to 12 hours prior to your blood draw. Take your regular morning medications with water unless directed otherwise.
Texas telehealth appointments
Before you connect.
- Physical Location Requirement
- Under Texas law, you must be physically located within the State of Texas at the time of your telehealth appointment.
- Device & Environment
- Connect from a private, well-lit, and quiet room using a secure internet connection and a computer, tablet, or smartphone equipped with a working camera and microphone.
- Sign-In
- Log into your telehealth session 5 to 10 minutes before your scheduled time to ensure your audio and video connections are working properly.
IV hydration preparation
- Drink plenty of water before your appointment to support comfortable vein access.
- Eat a light meal or snack within 2 hours prior to your infusion to help maintain stable blood sugar.

Nexus Mind & Medical Center is an outpatient medical practice and does not provide emergency medical services or 24-hour crisis intervention.
