How your information may be used

- To provide, coordinate, and manage your medical and behavioral health care, including sharing information with other clinicians involved in your care.
- Payment
- To bill and obtain payment from you, your insurance plan, or another responsible party for services rendered.
- Health care operations
- To support quality review, clinician training, compliance activities, and routine administration of the practice.
- As required by law
- To comply with federal, state, or local law, including public health reporting, health oversight, and judicial or law enforcement requests.
Your rights
Rights you hold over your record.
- Access and copies
- You may inspect and request a copy of your medical record, generally within 15 business days of a written request.
- Amendment
- You may request an amendment to information you believe is inaccurate or incomplete.
- Accounting of disclosures
- You may request a list of certain disclosures of your protected health information.
- Restrictions
- You may request restrictions on certain uses and disclosures, including disclosures to a health plan for services paid in full out of pocket.
- Confidential communications
- You may request that we contact you at a specific phone number or address.
- Complaints
- You may file a complaint with the practice or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.

What we commit to.
Nexus Mind & Medical Center, PLLC is required by law to maintain the privacy and security of your protected health information, to provide you with this notice of our legal duties and privacy practices, and to notify you promptly if a breach occurs that may have compromised the privacy or security of your information.
Most uses and disclosures for marketing purposes, and any sale of protected health information, require your written authorization. Psychotherapy notes, where they exist, receive heightened protection. You may revoke an authorization in writing at any time.
Clinical documentation is maintained in a HIPAA-compliant electronic health record, and patients communicate with the clinical team through a secure patient portal rather than through standard email or website forms.
